The service aims to support people living in Barking and Dagenham experiencing homelessness (street homeless, sofa surfing, squatting, facing eviction or living in a hostel/refuge/temporary accommodation) in accessing and navigating mainstream healthcare services and with appropriate self-management of any health conditions. With the support of MHBD staff and volunteer Peer Advocates, clients will be provided with ongoing practical and empathetic support to enhance their confidence and ability to manage their health independently.

Our Team will be able to support clients with the following:

  • Finding out about health services
  • Registering with GP and Dental practices
  • Booking healthcare appointments
  • Attending healthcare appointments e.g. Dentist, psychological therapies, substance misuse services, hospital outpatient appointments
  • Understanding and remember information that is given to them about their health and any treatment and healthcare appointments
  • Advocating at appointments to ensure the person’s voice is heard and that they understand health information that is given to them so they can make informed decisions about their care
